Middle/High School: $95.00
plus $25-MS / $35-HS

The following forms MUST be completed and returned before your player will be allowed to participate in Signal Mountain Lacrosse.

- Athlete Code of Conduct
- Parent Code of Conduct
- U.S. Lacrosse Membership Form
*NOTE: U.S. Lacrosse Membership ss required for all players for our league to meet insurance requirements. You will need to download the form above and submit it to U.S. Lacrosse, or you can register online here.
Uniforms & Equipment
Uniforms will be provided and must be worn at all games. Each player will also need the following equipment:
- Helmet
- Stick
- Shoulder pads
- Arm pads
- Gloves
- Mouth guard
- Cleats
